  7. PLACE OF BIRTH, Naming the State, Territory, or Country.
  8. THE MONTH in which the person died.
  9. PROFESSION, OCCUPPATION, OR TRADE.
  10. DISEASE OR CAUSE OF DEATH.
  11. NUMBER OF DAYS ILL.
